Welcome to Cooper Green Mercy Health Services Ambulatory Care. Our first priority is you. Please take a few minutes to read some very important information that will help ensure that you have the best possible experience during your visit at Cooper Green Mercy Health Services.

Our goal at Cooper Green Mercy Health Services is to assure that our clinics give you the most appropriate and specialized medical attention. We want to be your medical home and the first choice for residents of Jefferson County for receiving health care.

There are many ways our clinics can help you achieve and maintain good health. In addition to annual and follow up examinations, we provide valuable information on diseases such as heart disease, diabetes and its complications, and high blood pressure. We offer tips on how to be active, eat a proper diet and prevent or manage the disease.

 

 

  • Medical Home provides Primary Care, Internal and General Medicine.
  • Clinic B includes Ophthalmology, Dermatology and ENT services.
  • Clinic D cares for patients with needs related to Surgery, Orthopedics, Rheumatology, Urology, Neurology, Renal (Kidney) diseases and Podiatry.
  • Clinic E serves patients who have a specific blood disease and disorders and malignancies.
  • Pain Management Clinic provides a multidisciplinary approach to pain management.

Our clinics operate Monday through Friday between the hours of 8:00 am and 5:00 pm. The clinics are closed on Jefferson County holidays and weekends.

Appointments for the Medical Home can be made through our Appointment Center at 205.930.3350. Appointments for Clinics B, D, and Pain Management are by referral only.

Upon arriving at the clinic for your appointment, please visit the front desk so we can acknowledge your presence. You will soon be called according to your assigned clinic. Parking is available in the parking deck at the west end of the building.

We accept most major insurance policies including Blue Cross, Medicare, Medicaid, United Healthcare, and others. If you do not have insurance then the County may be able to provide financial assistance for your healthcare needs through our Health First Program. You must enroll in the Health First Program before receiving county assistance for any medical or pharmacy services at Cooper Green Mercy Health Services, so please plan ahead.

Our enrollment counselors at Cooper Green Mercy are looking forward to assisting you. They can answer any questions you may have regarding Health First enrollment, the first step to excellent healthcare for you and your family.

The Enrollment Office Hours are Monday through Friday, 7:00am to 2:00pm. If you are unable to come during those hours, please call (205) 930-3377 for an appointment.
